Returning families do NOT need to fill out a blank enrollment form. There will be forms mailed out to you that are pre-filled with information from PowerSchool. You will be asked to verify the information is correct. If there have been any changes in your household please mark the changes in red ink so we can update your information. If you left the district, for any amount of time, and are now returning; you will need to follow the New To District directions.
All new out of district students are reminded that a completed request for attendance must be on file with the school office by enrollment. Any out of district student who has not made this request should do so as soon as possible.
Out of District Request Form (PDF)If you have recently moved to the district, we will need the following information:
- Certified copy of birth certificate for all students
- Immunization Records
- Social Security Card
Please bring this information with you when you enroll your student(s) along with your student's enrollment form.

- Pre-Participation Physical Form (PDF) Students planning to participate in school sports MUST have a physical on file with the school before they can participate in any practice sessions. Plans should be made to get a physical and to have it turned in to the school office or coach prior to the first practice day on August 15th.
- KSHSAA CONCUSSION & HEAD INJURY INFORMATION/RELEASE FORM 2011-2012 (please note: This form must be signed by a parent and the student before they will be allowed to practice.)
