Pre-Participation Physical Form (PDF) Students planning to participate in school sports MUST have a physical on file with the school before they can participate in any practice sessions. Plans should be made to get a physical and to have it turned in to the school office or coach prior to the first practice day on August 13th.
KSHSAA CONCUSSION & HEAD INJURY INFORMATION/RELEASE FORM 2012-2013 (please note: This form must be signed by a parent and the student before they will be allowed to practice.)
