2020-2021 School Year Pre-Participation Physical, KSHSAA Concussion Form, Concussion Video, and Activity Fee Reminder:
If you have an SFTHS student-athlete that is planning on participating in a sport, please remember that the following requirements must be met before the first practice:
- A Pre-Participation Physical Form must be on file that clears the student for activities. The newest form must be used this year. There are copies of the Pre-Participation Physical Form available at Santa Fe Trail High School. In addition, the form is on the KSHSAA website at http://www.kshsaa.org/Public/

- The state required KSHSAA Concussion Form can also be obtained at the SFTHS office or accessed at http://www.kshsaa.org/Publ…/…/KSHSAAConcussionGuidelines.pdf. This form requires the student and parent/guardian signature as well.
- For grades 9th – 12th, the Santa Fe Trail School District requires a $50 Activity Fee for the 2020-2021 school year. This year’s fee and any previous year’s Activity Fee balances will be due if your student plans to participate and can be paid to the high school office. In order for your student to be able to participate in SFTHS sponsored activities, the SFT Activity Fee has to be paid and current prior to the first practice.
The KSHSAA pre-participation physical exam form has been updated based on the most recent edition of the PPE: Preparticipation Physical Evaluation, 5th Edition publication. The updated edition was developed by the American Academy of Pediatrics and other supporting organizations.

Medical Eligibility Page: This is the biggest change to be aware of regarding the new form. This is a new page at the end of the PPE paperwork that requires a second signature from the healthcare provider AND the parent/guardian. This page contains the student-athlete clearance, and parental consent for participation and emergency treatment. For a PPE to be valid when submitted to the school, healthcare providers are now required to sign two different places on the form (page 3 and page 4).
Healthcare providers must provide 2 signatures. Page 3 (physical exam page) and page 4 (medical eligibility page) both require the provider’s signature.
Parents/guardians must provide 3 signatures. Page 2 (health history page), page 4 (medical eligibility page) and page 5 (eligibility checklist).
Spanish Version: We are currently working to get the new form translated to Spanish. Right now the Spanish PPE form found online is still the previous version. The previous version of the PPE can still be used and accepted by schools during the 2020-2021 school year.
