All students participating in a KSHSAA sponsored activity, must have the following items on file with the office prior to practice or competition:

1. A completed Physical (must be dated after April 1st of the current school year) 
2. KSHSAA Concussion Course Certificate - Click Here KSHSAA Concussion Video Requirement
3. Concussion Form 
4. Orthopedics & Sports Medicine Consent
5. District Random Drug Testing Consent Form
6. District Activity Policy

KSHSAA

The Kansas State High School Activities Association (KSHSAA) serves students by providing leadership for the administration of education based interscholastic activities.

Gate Fees

• Adults: $7

• Non-District Students: $5

• USD 434 Students: FREE

• Children (5 & under): FREE

• District Activity Pass for Adults for $50. This pass can be used at all High School and Middle School Athletic events. If you would like to purchase a pass, please do so at the High School or CAC Office.
