Employee Wellness Program 2024 2025
Program Overview
This Program is available to ALL USD 434 District Employees. (Three Lakes teachers and paras working in the USD 434 buildings may participate in the program.)
Participating in the program is completely optional and reported is based on the honor system.
There are four levels available to complete if you choose to participate in the program and they must be completed in chronological order. Each level has different ways to complete. You can chose to participate in the minimum amount needed to complete Levels 1-2-3, which is completing 5 out of the 8 months challenges. However, to reach Level 4 you will need to complete ALL 8 months of challenges.
You can chose to start the program at any time during the year, however if you do not start in September you will not be able to complete the program in its entirety and be eligible for the Level 4 prize, but it is till an option.
You will return to this Google Worksheet each month to record and submit your data for the month. You can enter as you go and save, or enter all at the end of the month, your choice. Data is due at the end of each month, not the end of each level. Each months data will need to be entered by the 5th of the following month to receive credit and Level rewards will be sent out by the 15th.
